Transition to Adult Care: The RN Role in Improving Transplant Health Management Skills

This presentation will provide an overview of the transition of care (TOC) process, through the foundation of the Six Core Elements of Transition. The essential role of nursing in TOC will be explored and with discussion on how nurses can engage and educate adolescents and young adults in learning about and improving their self-management skills and increasing their independence.

Learning Objectives

After completing this activity, the learner will be able to:

  • Provide a general definition of transition of care and how it differs from transfer
  • List the Six Core Elements and provide at least one example of an activity from each element that relates to inpatient nursing care of the transitioning adolescent and young adult. 
  • Provide three strategies that can be used within a typical shift to promote a transition skill for your adolescent patient.
