Learning Objectives
At the completion of this training individuals should be able to recognize the benefits of naming a Health Care Power of Attorney and the importance of discussing end of life choices with their patients.
They should also be able to understand the differences/limitations between the Living Will, the POLST, and the Out of Hospital DNR form.
The participants will have the necessary skills to complete the POLST Form.
In addition, the health care professional will recognize and verbalize the process of completing these forms in facilities and community-based settings.

9:00 am to 10:00 am- Advance Care Planning Process
10am to 10:15am- Break
10:15am to 11:15am- Completion of Documents for Emergencies (POLST, Out of Hospital DNR)
11:15am to 12:15pm- Demonstrations
12:15 to 12:30pm- Wrap up, questions, evaluations
