Complex Care Team: A Collaborative Approach to Discharge Planning

Discharge Planners face challenges every day with complex patients needing resources when preparing for safe discharges from the hospital. These barriers often result in long length of stays and increased costs to the hospital. The Complex Care Team can assist with some of these challenges and barriers and facilitate more timely and safe discharges. This session will raise awareness to the resource available to them by referring to the Complex Care Team.

During this session, Discharge Planners will learn how the Complex Care Team can assist with difficult and complex discharges by breaking through barriers. Discharge Planners will also be able to recognize what common barriers would warrant a referral to the Complex Care Team and what interventions the Team could provide.

  • Be able to confidently identify examples of common barriers that would warrant a referral to the Complex Care Team.
  • Gain a general understanding of the referral sources and the types of interventions/resources that the Complex Care Team can help facilitate/provide.
  • Understand and be able to replicate the current referral processes.
