From Outpatient Care through Discharge Planning; An Overview of Delivering Care to Veterans at UPMC
Care coordination for veterans can be difficult, especially when we are unsure of who to call, what the veteran is eligible for, and how to coordinate ongoing care across the continuum. To expand on the briefing provided by the Department of Military and Veteran Affairs, we will look at advanced aspects of delivering veteran care, community care, eligibility, and many other aspects of veteran care.
After the presentation, Discharge Planners will have a better understanding on how to proceed with connecting Veterans to state and local resources that may have been difficult to do prior to attending.

In support of improving patient care, the University of Pittsburgh is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
As a Jointly Accredited Organization, University of Pittsburgh is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit.
