Hospice & Palliative Care: From Cure Focused to Comfort Centered
Transitioning patients from acute hospital care to hospice at home can be challenging for physicians, care teams, patients, and families. This educational program will explore how clinicians can recognize appropriate timing for hospice referral, navigate meaningful goals-of-care discussions, and coordinate effective discharge planning to ensure a smooth and supportive transition to hospice services.
