Community HealthChoices Overview

This learning activity provides a foundational overview of Pennsylvania’s Community HealthChoices (CHC) program. Participants will learn who is eligible for CHC and Long-Term Services and Supports, gain clarity on care coordination roles and responsibilities within the CHC model, and understand how these roles support participants throughout enrollment and ongoing services. The session also highlights key contacts and resources available to assist participants, providers, and care teams before and after CHC enrollment, promoting effective communication and continuity of care.
