Overview
Target Audience
This program has been designed for Physicians, Physician Assistants, Nurse Practitioners, Nurses and other healthcare professionals who are involved with End of Life care.
Learning Objectives
As a result of participation in this activity, participants will be able to:
- Discuss useful tools to assist in facilitation of POLST conversations
- Identify key sections of POLST form and scripts for discussing with patients and families
- Review helpful phrases and guides to facilitate POLST conversations.
