Board certification and state license (PHARMACISTS AND PHYSICIANS ONLY) PHARMACISTS: Select “National Association of Boards of Pharmacy” in the “Specialty Board or State License” field. The NABP e-profile ID (e-PID) MUST be included in the e-PID field for credit to be reported. The Date of Birth field below MUST also be completed for credit to be reported. DO NOT add your State or State License ID number. PHYSICIANS: Select the state you are licensed in and supply your corresponding license ID number. DO NOT enter if you have a temporary or trainee license. State and License ID number MUST be included for applicable credit to be reported. Additionally, for Physicians with American Board of Medical Specialties Certification, add the appropriate Board and supply your corresponding Diplomate ID. The Board and Diplomate ID MUST both be included for applicable credit to be reported. The Date of Birth field below MUST also be completed for applicable credit to be reported.
