Prescription for Wellness: Supercharging Wellness Engagement and Outcomes through Provider Referral
This presentation will focus on strategies to leverage insurance carrier resources including Wellness Services and Digital Tools specifically through Prescription for Wellness, an integrated platform at UPMC Health Plan.
Target Audience
Providers that are treating patients that are UPMC Health Plan members and are interested in learning more about lifestyle medicine and Prescription for Wellness.
This course is designed for physicians, APP, nurses, and clinical staff.
Learning Objectives
Upon completion of this activity, participants should be able to:
- Describe strategies they can deploy to integrate care across payer and provider with the goal of improving population health outcomes and addressing health care disparities.
- Explain how to utilize quality measures and data analytics to evaluate the impact of provider-prescribed health coaching and care management on patient-member health, self-care for chronic diseases, and shared decision-making.
- Define best practice processes to promote strategic implementation and optimization of a patient-member engagement platform.
