Journal Critique Self Study: Effect of Age-Friendly Care on days at Home Post Hospital Discharge
Learners will read the article “Effect of Age-Friendly Care on Days at Home Post-Hospital Discharge for Traditional Medicare Patients” and respond to ten guided questions that deepen comprehension and encourage critical application of the 4Ms framework in nursing practice. This activity addresses gaps in age-friendly care by helping nurses move beyond assessment-only approaches toward integrated, evidence-based interventions that prioritize patient-centered outcomes such as “days at home.”

Learning Objectives
Upon completion of this activity, participants should be able to:
- Describe how the 4Ms framework (What Matters, Medication, Mentation, Mobility) is applied in hospital settings and its impact on patient-centered outcomes such as “days at home.”
- Evaluate how assessment-level nursing interventions influence patient recovery trajectories in the first 30 and 90 days after hospitalization, including facility utilization and home recovery.
- Recognize gaps in nursing practice (assessment vs. action, fragmented care across settings) and propose strategies for integrating Age-Friendly care across hospital, post-acute, and community environments to enhance patient independence and reduce readmissions.
