This training is designed to provide knowledge in best practice for conducting Medicare Annual Wellness in person visit and synchronous audio video telehealth Lack of direction in available resources to assist with implementing telehealth visits and best practices for conducting AWV and functional/safety/cognitive and safety assessments.

Primary care physicians and physician extenders.

Upon completion of this activity, participants should be able to:
- Describe the use and application of telehealth and telemedicine in caring for patients
- Direct physicians and other clinicians to available resources to assist with the implementation of telehealth
- Inform of best practices for conducting a Medicare Annual Wellness Visit both for in person and synchronous audio video visits
- Direct physicians to available resources and best practices for conducting functional, safety and cognitive assessments and depressions screenings
