5 C's of Communication

Discharge Planners come from various backgrounds, some with little or no experience communicating with Providers, Patients, and Families. Effective communication helps to build a rapport and trust with these individuals and this education session will provide tools to help them be more confident and techniques on how to implement these skills.

More effective communication techniques will enhance the trust and rapport among the interdisciplinary team. This may also streamline the overall discharge planning process, which benefits not only the team but the facility and patients/families.
