This 1-hour training session continues out Serious Illness Series with a review of advanced care planning documents and how to effectively communicate these options to health plan members.

Learning Objectives
- Participants will be able to describe the similarities and differences among advanced care planning documents including living wills, advanced directives, POLSTs, and out of hospital DNR forms
- Participants will be able to respond to emotion with verbal empathy using NURSE (Name, Understand, Respect, Support, Explore) statements
- Participants will be able to describe and practice motivational interviewing techniques of opened questions and affirmations when discussing advanced care planning
