This presentation focuses on enhancing support for UPMC Health Plan members with heart failure through improved collaboration between payors and providers. It covers key topics such as heart failure overview, targeted automated eConsults, and HVI heart failure clinics

  • Recall the different types and classifications of heart failure, including left ventricular and right-sided heart failure, as well as heart failure with reduced ejection fraction (HFrEF) and heart failure with preserved ejection fraction (HFpEF) and Recognize symptoms of heart failure, such as shortness of breath, fatigue, swollen legs, and rapid heart beat, and  the importance of medication adherence, dietary changes, and physical activity in managing heart failure 

  • Describe the current treatment recommendations and guideline-directed medical therapy (GDMT) for heart failure, including the use of beta-blockers, ARNI, MRAs, and SGLT-2 inhibitors and the role of targeted automated eConsults (TACOs) in heart failure management and the structure of the Medication Optimization Clinic . 

  • Apply the knowledge of heart failure self-management strategies, including medication adherence, dietary changes, and physical activity, to improve patient outcomes and understanding of the economic impact of heart failure to advocate for cost-effective treatment and prevention strategie
