ASCP- Pennsylvania Spring Education Day

Pharmacists work in various care environments such as community pharmacy, hospitals, clinics and long-term care. In addition to being positioned to work with patients and caregivers directly, pharmacists are an integral part of the interprofessional healthcare team with a goal to optimize pharmacotherapy use in older adults. This activity will increase the pharmacist participants' knowledge and skills in immunizations, pain management and fall prevention to enable them to design improvements in care plans with the interprofessional healthcare teams in which they participate.

Learning Objectives

Session 1: 2024 Immunization Update (Dr. Kryger and Dr. Milito)

1. Explain the vaccine approval process in the Unites States

2. Examine federal and state (Pennsylvania) regulations pertaining to pharmacist administration of immunizations

3. Identify nursing facility regulations & quality measures related to immunizations and how to incorporate vaccination into the antimicrobial stewardship program

4. Discuss the CDC immunization schedule recommendations for people over the age of 65

5. Analyze case-based examples to determine vaccine eligibility

Session 2: Opioid Stewardship Principles: Managing Pain in Older Adults across the Care Continuum (Dr, Lowry)

1. Review opioid equianalgesic table and its utility for opioid conversions across the continuum of care

2. Describe three clinical pearls regarding opioid considerations in the geriatric population

3. Discuss impact of age-related pharmacokinetic changes on dosing and selection of opioids in the geriatric population

4. Analyze considerations for adequate opioid management across the continuum of care

5. Apply learned opioid concepts to patient case examples

Session 3: Pharmacists Defying Gravity: Fall Assessment and Prevention in Older Adults (Dr. O'Neil)

1. Discuss the role of comprehensive falls risk assessment and intervention in their practice utilizing falls prevention toolkits from the Centers for Disease Control and Prevention’s Stopping Elderly Accidents, Deaths, and Injuries (STEADI) initiative.

2. Identify common risk factors for falls in the senior population.

3. Identify medication related risk factors for falls. 

4. Utilizing patient cases, develop individualized recommendations for fall prevention based on a medication review.
