Bridges for Transition
The focus of this webinar will be on identifying tools to assist in the successful transition process to adult care for adolescents with chronic conditions, their families and caregivers and the multidisciplinary providers that care for them.
Target Audience
Patients with chronic medical and developmental conditions, their parents and caregivers as well as practitioners interested in positive development of youth with medical challenges and disabilities to receive appropriate transition care education.
Learning Objectives
The goal of this webinar is to identify the legal concerns for patients with neurodevelopmental disabilities, to understand the needs of patients with chronic medical conditions for transition to adult care, be able to conduct a transition interview and assessments, and understand how to bill for use of validated transition tools.
