9:00 a.m. to 9:10 a.m. – Welcome, Introductions and Review of Survey Results – Robert Ferguson, MPH, Chief Policy Officer, Jewish Healthcare Foundation

 In preparation for this webinar, we will survey the HealthChoices PCMHs to learn which HIO functions they are using (e.g. encounter notifications) and how they have incorporated these functions in their workflows and team roles. Robert will review the survey findings at the beginning of the webinar.

9:10 a.m. to 9:40 a.m. – Connecting to the HIOs - Martin Ciccocioppo MBA MHA, Director, Pennsylvania eHealth Partnership Program, Pennsylvania Department of Human Services

 Martin will review how connecting to the Health Information Organizations (HIOs) enables PCMHs to receive and follow-up on encounter notifications.

 Martin will also provide an update on the status of onboarding outpatient practices and reinforce the guidance that only P3N HIOs qualify for the following HealthChoices PCMH provider expectation: “Will join a Pennsylvania Patient and Provider Network (P3N) certified health information organization (HIO) by 12/31/2020 in order to share health related data.”
