Age Friendly Health Systems: 4Ms and Mercy

UPMC Mercy is committed to providing excellence in the care of older adults.  The 4Ms of the Age Friendly framework developed by the Institute for Healthcare Improvement (IHI) provides the map to guide UPMC Mercy in this journey. This program will provide an overview of what an Age Friendly Health System means and how the 4Ms are relevant to the work of nursing and care management.

  • Define an Age Friendly Health System
  • Identify the 4Ms of Age Friendly Health Systems
  • Recognize the role in care management and nursing in providing age friendly care by supporting the 4Ms.
