CHP Community Partner Spotligh self learn

Basic needs such as transportation access, food insecurity, and stable housing are just a few of the social determinants that, when unmet, adversely impact the health and wellbeing of children and their families. While these basic needs are necessary to achieving positive health outcomes, the medical community is often unaware of the resources available in their patients’ communities. The Division of Community Health aims to increase awareness of key community partners and the resources they provide while prioritizing efficient mechanisms of referral to these and other community resources.
