Collaborative Care Model

This course provides medical professionals with the background and evidence-base rationale for implementing integrated care in primary care practices to address their patients’ behavioral health needs. The course educates providers specifically on the Collaborative Care Model’s core concepts and components. A case example is utilized to further demonstrate the benefits of providing integrated care. The course also addresses challenges and barriers in implementing Collaborative Care in primary care settings.

There is no educational content to this course. This course is designed to supplement training that has already occurred. This course is designed to only administer the post-test and collect CME evaluations.

  • Understand the importance of treating behavioral health issues within the primary care setting.
  • Incorporate the core principles of integrated care/collaborative care into routine care.
  • Identify the most common barriers and key ingredients to behavioral health integration.
  • Capture integrated behavioral health activities in specialized CPT billing codes.
