Attendees will learn the nurse's role in successfully preparing a patient for discharge including how to perform diabetes education which includes: blood glucose monitoring, carbohydrate counting, insulin administration, impact of exercise, & hyper/hypoglycemia.

Learning Objectives

Upon completion of this program the learner will be able to:

1. State the mission and goal of Diabetes Boot Camp.

2. Discuss the information a nurse must know to prepare a patient with diabetes for a safe discharge.

3. Explain the impact exercise/activity has on blood glucose levels.

4. Review the steps involved in teaching patients how to perform self-blood glucose monitoring at home.

5. Demonstrate how to both count carbohydrates and to dose insulin using carbohydrate counting.

6. Discuss the best practices for insulin administration during hospitalization and for patients at home

 7. Differentiate between symptoms of hyperglycemia and hypoglycemia.

8. Explain proper response to hypoglycemia using the Rule of 15.
