This in-service will provide nurses the understanding of how the home care process works, how to identify barriers, and how to work through those barriers to improve the plan of care in preparation for discharge
The learner will gain a high level understanding of home care definitions and processes
The learner will be prepared to locate documentation of home care status in the electronic medical record
The learner will have an understanding of current barriers to home care and the effects on discharge planning
