During the first quarter of 2023, the focus has been Age-Friendly Health Systems for Nursing Facilities and how the 4Ms Framework (what matters, medications, mobility, and mentation) can be applied to a care setting. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).  The Pennsylvania Long-Term Care Learning Collaborative
• Will feature several best practices from nursing facilities across the state
• Team members from the Institute for Healthcare Improvement will discuss next steps
• Updates will be provided from the Department of Human Services | Office of Long-Term Living and the Department of Health
