Person-Centered Approach to Social Determinants of Health (SDoH) Assessment_Enduring
This self-paced Motivational Interviewing skills building will support staff with a model for using person-centered skills when exploring Social Determinants of Health with members and using the Social Determinants of Health Assessment. Learners will then review case studies and practice using common case scenarios in their setting with practice and application.
SPECIAL NOTE:
There is no educational content to this course. This course is designed to supplement training that has already occurred. This course is designed to only administer the post-test and collect CME evaluations.

Nurses, Social Workers, Health Coaches, Registered Dieticians, Pharmacists, and Paramedics / EMTs
Learning Objectives
- Name the Social Determinants of Health and the benefits of using a person-centered approach in administering the Social Determinants of Health Assessment
- Recall key Motivational Interviewing skills that can assist with administering assessments in a person-centered way
- Review a case study demonstration and discuss opportunities for a person-centered approach
Other health care professionals will receive a certificate of attendance confirming the number of contact hours commensurate with the extent of participation in this activity.
