Please join us for a live, CME-accredited webinar Tuesday, December 17, from noon to 1 p.m. that will provide a discussion about aggregate quality data as well as education of different fidelity guidelines and implementation strategies.
Target Audience

Learning Objectives
Upon completion of this activity, participants should be able to:
1. Review SDOH and the need for screening
2. Discuss barriers and challenges to addressing SDOH
3. Identify strategies to improve SDOH deficits
