Learning Objectives

After completing the training session, the health care professional will be able to:

Recognize the benefits of naming a Health Care Power of Attorney and the importance of discussing end of life choices with them,

Discuss the differences/limitations between the Living Will and the POLST form and will have the skills necessary to complete the POLST form.

Recognize and verbalize the process of completing and recognizing these forms in facilities and in the community.

11:15am to 12:15pm- Putting it all Together- Role Play

12:15 to 12:30pm- Wrap up, questions, evaluations
