A goal of the School Nurse Advisory Group is to help improve communication and care across all aspects of the healthcare team to improve children’s health. This activity will make an impact on the interprofessional healthcare team by developing the foundation for the development of a framework between school health professionals, hospital clinicians, and primary care.

- Following the meeting, the learner will be able to explain the role of the School Nurse Navigator in care management for students with chronic health conditions.
- Following the meeting, the learner will be able to name two ways that the School Nurse Navigator can help with case management for students/patients with asthma and diabetes.
