School Nurse Advisory Group - November 2024
This particular session is focusing on the Healthy Allegheny Teen Survey, School Care Connect, and clinical updates from the hospital. The nurses will learn more about recent trends in youth data and learn how to apply that to their school. The group will also discuss School Care Connect, a program designed to help increase communication and care coordination between school nurses and clinical providers. They will help discuss clinics to roll the program out to and ways to advance the program. Finally, the nurses will receive clinical updates on what is trending in pediatrics at the hospital currently.

After this meeting the school nurses will be able to describe Community Health's new School Nurse Navigator role and how the School Nurse Navigator can help with case management for students/patients with asthma and diabetes.
