This activity will provide the pharmacists a comprehensive review of literature surrounding the use of direct oral anticoagulants, as well as the guideline recommendations. Pharmacists will be equipped to make an educated decision and appropriate interventions for specific patient cases

Learning Objectives
1. Identify mechanisms of action and unique characteristics of each direct oral anticoagulant (DOAC)
2. Compare the recommendations for use of direct oral anticoagulants in the treatment of venous thromboembolism and stroke prophylaxis
3. Evaluate patient-specific factors to guide anticoagulant therapy decisions for various disease states
4. Formulate a reversal strategy for each direct oral anticoagulant
