The purpose of this educational activity is for UPMC Health Plan clinical staff to explore and better understand behavior change and coaching strategies for members with diabetes and prediabetes.

  1. Describe the importance of behavior change in diabetes management

PHARMACY OBJECTIVES:

  1. Discuss role of patient-directed behavior changes to improve underlying diabetes management
  2. Review how behavior changes can impact home adherence and understanding to diabetic medication regimens

Clinical Health Coach Specialist

Janine has a BS in medical dietetics and a M. Ed in nutrition.   She has 30 years’ experience working as a dietitian in diverse health care and community settings.  Janine has practiced in long term care facilities, rehabilitation centers, urban teaching-research hospitals, and small rural hospitals.  She has supplemented her clinical background with public health consultation, outpatient counseling, lifestyle health coaching, and facilitation of support groups.  In addition, she has collaborated with multidisciplinary teams for program development, marketing, and education in her positions. 

Janine has focused her career on facilitating individuals and groups with lifestyle change to meet their personal health goals through nutrition counseling, diabetes education, and lifestyle health coaching. 

She has been a Certified Diabetes Care and Education Specialist since 2003 and a health and wellness coach for 15 years at UPMC Health Plan.  As a clinical health coach, Janine applies client centered behavior change principles and motivational interviewing skills to help clients make healthy lifestyle changes.

 

UPMC Health Plan, Clinical Engagement Coordinator

Indira Stokes is a Clinical Engagement Coordinator at UPMC Health Plan where she is responsible for training, empowering, and the skill development of health coaches and care managers.  She has a bachelor's in Biology and Economics from the University of Pittsburgh and a master's in Public Health, specializing in behavioral and community health from the University of Pittsburgh. Indira is a Certified Health Education Specialist and a Nationally Board-Certified Health and Wellness Coach with over 5 years’ experience in health coaching.

 

Valerie  Vidic, RD, LDN, CDCES
Diabetes Care & Education Program Coordinator

Valerie Vidic is a Registered Dietitian and Certified Diabetes Care and Education Specialist. Valerie has 25+ years of experience providing diabetes self-management care and education in both the clinical and industry settings.  She worked as a clinical manager and technical support clinician for an insulin pump product company.  She worked as a key account manager in a diabetes self-test diagnostics market, and most recently with UPMC. Her career with UPMC started at Magee Women’s Hospital, Maternal Fetal Medicine, in 2015. Transitioning to Community Medicine (CMI), and then joining the UPMC Health plan in 2019 as a senior diabetes educator. She currently serves as the Diabetes Care and Education Program Coordinator.

In support of improving patient care, the University of Pittsburgh is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Pharmacy (CPE)
This knowledge-based activity provides 1.5 contact hours of continuing pharmacy education credit(s).
Nursing (CNE)
The maximum number of hours awarded for this Continuing Nursing Education activity is 1.5 contact hour(s).

CCM CE will be provided:1.5 CE - CCM COA will be sent via email from UPMC Health Plan nurse planner approximately 6-8 weeks following the live date of the course.

This activity is eligible for endorsed credit for UPMC Health Plan EMTs and Paramedics.  Complete ETHOS course work, obtain the attendance certificate, and submit to EMS governing body for 1.5 CE credit(s).

As a Jointly Accredited Organization, University of Pittsburgh is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. University of Pittsburgh maintains responsibility for this course. Social workers completing this course receive 1.5 continuing education credit(s).

Other health care professionals will receive a certificate of attendance confirming the number of contact hours commensurate with the extent of participation in this activity.
