The purpose of the educational activity is to provide the clinical staff with an update on the 2022 ACC/AHA/HFSA Heart Failure Guideline, review heart failure performance data, and to discuss implementation of the heart failure guidelines into clinical practice.

Dr. DeGregorio is an internal medicine physician serving as the AVP for UPMC for You. He completed medical school training at the University of Cincinnati School of Medicine, an Internal Medicine Internship and Residency program at the University of Pittsburgh, and a Master of Medical Management degree from Carnegie Mellon University. He is a past President of Preferred Primary Care Physicians with 20 of clinical practice experience. One of his key areas of interest, relevant to this presentation, is translating evidence-based guidelines into practice through population health and performance improvement programs.

 

Ashley Modany, PharmD
Sr. Clinical Pharmacy Specialist, UPMC Health Plan

Dr. Modany is a Senior Clinical Pharmacy Specialist in Pharmacy Care Management and currently serves as a Residency Coordinator for the PGY1 Managed Care Pharmacy Residency here at the health plan.  She attended Duquesne University School of Pharmacy and went on to do a PGY1 Acute Care Pharmacy Residency at Allegheny General Hospital, a PGY2 Cardiology Pharmacy Residency at UPMC Presbyterian Hospital, and an Advanced Clinical Simulation Fellowship at the VA Pittsburgh Hospital System. Prior to working at UPMC Health Plan she was a Heart Failure/Pulmonary Hypertension clinical pharmacy specialist at WVU Ruby Memorial Hospital. She has been with UPMC Health Plan for 5 years.

As a Jointly Accredited Organization, University of Pittsburgh is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program. State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit.
