ForwardKELS and MoCA...What They Are and What You Need to Know for Discharge Planning 7.25.24
Staff do not feel they have the knowledge/understanding of what KELS/MoCA assessments are and how to apply them to discharge planning, as identified in a Needs Assessment completed by the department.
Increased understanding of the KELS and MoCA among our discharge planning staff will lead to better communication amongst the care team and families in regard to helping families navigate patients to the most appropriate level of care (increasing patient satisfaction and decreasing length of stay within the hospital).

Learning Objectives
- Explain the differences between KELS and MoCA testing.
- Understand the strengths and limitations of KELS and MoCA testing.
- Explain 3 ways that the KELS or MoCA score can assist with discharge planning.
