This session will focus on well-described methods designed to accomplish patient-centered care in the context of situations where medical evidence supports specific behavior changes, and the most appropriate action is dependent on the patient’s preferences. Case scenarios will be reviewed with practice and opportunity for feedback.

1.  Identify and review the process of shared decision making in patient-centered care

2.  Recall basic principles of behavior change and how the motivational interviewing (MI) approach can support engaging members in shared decision making

Renee Greenwald is a Clinical Engagement Coordinator at UPMC Health Plan where she is responsible for training, empowering, and the skill development of health coaches and care managers.   She has a bachelor’s in Exercise Science from Slippery Rock University and a master’s in Exercise Science and Health Promotion, specializing in wellness and fitness and wellness coaching from California University of Pennsylvania.  Renee is a Nationally Board-Certified Health and Wellness Coach with about 5 years’ experience in health coaching.

Indira Stokes is a Clinical Engagement Coordinator at UPMC Health Plan where she is responsible for training, empowering, and the skill development of health coaches and care managers.  She has a bachelor's in Biology and Economics from the University of Pittsburgh and a master's in Public Health, specializing in behavioral and community health from the University of Pittsburgh. Indira is a Certified Health Education Specialist and a Nationally Board-Certified Health and Wellness Coach with over 5 years’ experience in health coaching.

 

In support of improving patient care, the University of Pittsburgh is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

CCM CE will be provided:  2 CE and sent from UPMC Health Plan nurse planner

As a Jointly Accredited Organization, University of Pittsburgh is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved under this program.
