GoHealth and UPMCHP: Diabetes Documentation and ICD-10-CM Coding

This session provides a brief overview of documenting diabetes for severity of illness by type and with complications to accurately report ICD-10-CM codes.

Target Audience

Physicians, Advanced Practice Practitioners (CRNP and PA), Residents, Medical Students, and Physician Practice Staff

Learning Objectives

Upon completion of this activity, participants should be able to:

  1. Describe the main diabetes mellitus (DM) code categories for compliant provider documentation to report the intended diabetes diagnosis for their patient.
  2. Recognize the ICD-10-CM code options to report diabetes by type to improve non-specific provider documentation for diabetic type.
  3. Apply basic ICD-10-CM coding guidelines to assist providers with compliant documentation to accurately report diabetic complications for their patients.
  4. Explain how diabetic codes reflect severity of illness based on hierarchical condition categories (HCC) used in risk adjustment models.
