OAPP Hospice Isn't Giving Up: Aligning Care with What Matters When Time is Short

Learning Objectives
-Describe hospice as an insurance benefit
-Identify basic eligibility criteria and differentiate between levels of hospice care.
-Emphasize appropriate use of each level of care based on CMS regulation.
-Articulate hospice philosophy in a clinical context:
-Explain why certain interventions are often discontinued at end of life even in the setting of patient preference.
-Identify important differences between hospice providers and available resources.
-Describe how hospice plans of care are individualized.
-Apply the principles of individualized care planning during IDT discussion
-Identify the participants of the IDT and their contribution to creating a hospice plan of care.
-List opportunities for providers to contribute meaningfully to a hospice plan of care prior to hospice admission.
-Communicate about hospice when the conversation feels hard.
- Practice language that introduces hospice honestly, and compassionately in the setting of prognostic uncertainty, emotional resistance or decisional conflict.
-Discuss strategies to dispel myths about hospice during family meetings.
