Orthopedic Care across the continuum for elective joint replacement and spinal fusion patients
Patients undergoing elective joint replacement and spinal fusion surgical procedures require specialized preoperative instructions from the pre-admission testing (PAT) surgical optimization clinic (SOC) nurses that are consistent with the instructions provided by the surgeon office. These instructions are often not available to be viewed by the PAT and SOC nurses. Review and explanation of preoperative instructions provided by the surgical office team will facilitate consistent and improved care of these patients across the continuum.
Improved communication, education and collaboration between the interprofessional team healthcare team facilitates better surgical outcomes. Review of patient care instructions specific to elective joint replacements and spinal fusion surgeries will increase nursing knowledge and promote communication and consistency of care.

Learning Objectives
PAT/ SOC nurses will be able to tailor the patient preoperative medication, diet and infection control instructions to the patient and specific procedure.
PAT/SOC will perform more accurate chart reviews for the surgical team.
