PA POLST Module 1: Introduction to POLST​

Target Audience

This program has been designed for Physicians, Physician Assistants, Nurse Practitioners, Nurses and other healthcare professionals who are involved with End of Life care.

Learning Objectives

As a result of participation in this activity, participants will be able to:​

  • Review of the history and background of advance directives and POLST in Pennsylvania. ​
  • Discuss information about Advance Health Care Directives since POLST works best when someone has selected the person who would speak for them if they become unable to make their own medical decisions. ​
  • Describe key aspects of POLST and the POLST form. ​
  • Outline teaching materials which attendees can use across all health care settings. ​
