Palliative Care and Hospice
Hospice and Palliative Care are services that are available to our patients in the hospital setting and Discharge Planners play a pivotal role in assisting with making arrangements for this type of care. During this session, Discharge Planners will learn the differences between Hospice and Palliative Care and how they can benefit patients.
Discussing Hospice and Palliative Care can be a difficult conversation for many Discharge Planners. This session will help understand not only the difference between the two but how to engage in the conversation and make arrangement for these services.

In support of improving patient care, the University of Pittsburgh is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
