PCMHs will learn about effective tactics and strategies for training and supporting and reinforcing staff in motivational interviewing and trauma informed care to help enhance the effectiveness of SDOH screenings for patients. The session will also include information on training, onboarding and supervising Community Health Workers, with a spotlight on how one Pennsylvnia PCMH has approached integrating and supporting CHWs as members of the multidisciplinary care team.
