    • On your PCMH team, whose role is to help your patients engage in mental health treatment?
    • What activities does that team member perform to engage patients in mental health treatment?
    • What quality measures do you track to continuously improve processes related to identifying patients with mental health needs, engaging them in treatment, and ensuring their treatment is successful?
    • How is your practice sustaining these team roles, activities, and improvements? Are you using the collaborative care management codes (99492, 99493, 99494), the general integration code (99484), HBAI codes, or other billing codes?

The presentation will cover workflows for the collaborative care management model and how to bill 99492, 99493, and 99494 in different types of practices (medical groups/physician offices, FQHCs, and pediatric practice).

10:30 a.m. to 11:25 a.m. – Applying Empathetic Inquiry Skills to SDOH Screening and Follow-up Processes During a Primary Care Office and Telehealth Visit – Bruce Block, MD, Consultant, PRHI, Carol Frazer, MEd, LPC, Practice Transformation Specialist, PRHI

The presentation will cover how nurses, medical assistants, and other primary care providers can apply empathetic inquiry principles and skills when introducing, administering, and offering feedback on SDOH screens during a routine primary care office or telehealth visit.
