PCMH Southeastern PA Virtual Learning Session 4/22
The PCMH practices completed a Learning Needs Assessment and they indicated they lacked competence and knowledge around completing a SDOH assessment using a nationally recognized tool and submitting diagnosis codes and also deploying a community based management team consisting of nurses, social workers, community health workers and medical assistants to engage the patient in developing a personalized care plan and connecting them to community resources and social support.

- Describe effective approaches for screening and connecting patients to social supports while exercising precautions for the spread of viruses, including the coronavirus.
- Describe the structure of Motivational Interviewing for engaging patients.
- Describe ways to incorporate motivational interviewing to engage and activate patients in their care.
- Identify how an HIO is sharing patient encounters with health care providers.
