PCMH Western PA Learning Session 2.1.22

The PCMHs need information on how to follow up once health equity metrics are known to better understand the root causes of disparities and address those disparities. We will present to the PCMHs their role in working with their Regional Accountable Health Councils (RAHCs) as a source of discovering strategic community-wide efforts to improve health outcomes across the state. We will describe to the PCMHs what constitutes as language access and cover the best practices for language access resources.

8:30 a.m. to 8:35 a.m. – Welcome & Introductions – Robert Ferguson, MPH, Chief Policy Officer, Pittsburgh Regional Health Initiative (PRHI)

8:35 a.m. to 9:25 a.m. – Presentation by the Regional Accountable Health Council (RAHC) – Dr. James Schuster, UPMC Community Care Behavioral Health and Julie Evans, MHA, PMP, Manager, ProspHire

    Dr. Schuster will present an update on the RAHCs equity plans and goals and how this intersects     with a PCMHs workflow to follow up on positive SDOH screens and connecting patients to community resources.

9:25 a.m. to 10:15 a.m. – PCMH and MCO Report Outs– Facilitated by Pauline Taylor, Program Specialist, PRHI

Each MCO will report out on the following:
⦁    When you look at your PCMH-related outcome metrics by race, ethnicity, geography, and language, what are the major disparities in outcomes?
⦁    How are you working with the PCMHs in your network to close these gaps in disparities based on the root causes?

10:55 to 11:25 am - MCO-Specific Breakouts 

The facilitated breakout groups will discuss:

⦁    How can the MCO help PCMHs use the MCO’s data feedback reports in a meaningful way to assess their progress and drive improvement, especially in terms of closing gaps in disparities?
⦁    What is the best way to display and view the MCO data from the perspective of a PCMH? What’s worked well? What hasn’t worked well?
⦁    How do you use both the MCO and PCMH level data for a complete picture to understand the disparities and close the gaps?

11:25 a.m. to 11:30 a.m. – Wrap-Up & Next Steps – Pauline Taylor
