Pharmacy continues to evolve and with that comes practices changes, new challenges and areas for improvement. ISMP continues to update their safety recommendations and Pennsylvania state board of pharmacy requires that all pharmacists have 2 patient safety contact hours for license renewal.

Learning Objectives
Using a patient case, determine important pharmacist interventions with regards to warfarin dose adjustments, monitoring, or reversal
Utilize 2020 Joint Commission National Patient Safety Goals (NPSG) as the basis for a prospective pharmacist driven warfarin protocol
Review current UPMC institutional guidelines on pharmacists’ role for safe warfarin management
