School Partnered Collaborative Care (SPACE) - February 2026 Part A

School nurses are participating in student-specific meetings with a certified diabetes care and education specialist to review diabetes management and education needs related to diabetes devices, nutrition, and care coordination, among other topics.

  1. Describe updates to diabetes management, including expectations with new diabetes devices (e.g., routine use, managing hyperglycemia, managing hypoglycemia).
  2. Understand the importance of care coordination between schools and health systems.
  3.  Adopt new strategies to improve diabetes management in school
