School Partnered Collaborative Care (SPACE) - February 2026 Part A
School nurses are participating in student-specific meetings with a certified diabetes care and education specialist to review diabetes management and education needs related to diabetes devices, nutrition, and care coordination, among other topics.

- Describe updates to diabetes management, including expectations with new diabetes devices (e.g., routine use, managing hyperglycemia, managing hypoglycemia).
- Understand the importance of care coordination between schools and health systems.
- Adopt new strategies to improve diabetes management in school
