School Partnered Collaborative Care (SPACE) - January 2025

Children with type 1 diabetes receive a great deal of their management in school under the care and supervision of school nurses. Presently, school nurses follow diabetes medical orders from the child's treating diabetes provider. However, the relationship between school nurses and the medical team is otherwise highly variable, and typically these groups work in silos. One of the primary goals of this activity is to create a virtual space for school nurses and a member of the diabetes medical team to meet and discuss aspects of diabetes management, personalized to the student's health and education needs. We anticipate that participation in this activity will foster more effective communication between these different groups.

After completion of the program the learner will be able to:

  1. List who to contact in the diabetes medical team.
  2. Communicate diabetes management needs effectively to the diabetes medical team and family.
  3. Use person-centered language when discussing diabetes care. 
  4. Describe different types of diabetes technologies and demonstrate appropriate use.
