This session will help Discharge Planning professionals understand the background, importance and the value of screening for Social Factors and have a greater understanding of the Social Determinants of Health.
This session will help DPM's understand the importance of developing and providing resource information and connect patients/family to community resources
After this session DPM's will have a greater understanding of the Social Determinants of Health and how they impact discharge planning and readmission prevention.
Discharge planners will be better able to assist their patients with needs that have been determined by the SDoH assessment.
