Recently identified safe medication practice opportunities for which pharmacist education and engagement is needed to promote best practices. Pharmacists will learn practical strategies for avoidance of potential medication-related events, as well as ways to eliminate or mitigate preventable patient harm possibly related to medication use.
Target Audience
Pharmacists from the University of Pittsburgh School of Pharmacy and/or employed by UPMC and deployed throughout the hospital campus in unit based roles and centrally in the department of pharmacy's main pharmacy.

- Describe one recently reported medication event that occurred at our hospital.
- Discuss what a pharmacist should do to prevent a similar event and/or mitigate patient injury caused by this type of medication safety issue.
