UPMC ALT Clinical Care Coordination and Discharge Planning Staff Education
This education is being provided to UPMC employees who are transitioning to the new role of Discharge Planner to create a foundation of knowledge and skills necessary to perform successfully within this new role.

- State how to determine surrogate decision makers in accordance with state law
- Identify steps to find next of kin Describe when to consider guardianship
- Identify levels of care for behavioral health
- Identify when it’s appropriate to complete PA OPTIONS process
- Understand what conditions will require a Level II assessment in PA
- Describe the three main types of admission under a PA PASRR
- Identify warning signs for suspected abuse and neglect
- Understand abuse reporting requirements
- Identify documentation requirements for child abuse reporting
