Learning Objectives
Identify strategies to enhance communication and collaboration among healthcare providers, patients, families, and support networks to improve the transition journey from pediatric care to adulthood.
Develop skills to design comprehensive and individualized transition plans that address the medical, emotional, social, and functional needs of patients with chronic childhood conditions as they move into adult care.
