Learning Objectives

Identify strategies to enhance communication and collaboration among healthcare providers, patients, families, and support networks to improve the transition journey from pediatric care to adulthood.

Develop skills to design comprehensive and individualized transition plans that address the medical, emotional, social, and functional needs of patients with chronic childhood conditions as they move into adult care.

Recognize common challenges faced by transitioning patients and families, including legal, financial, and technological barriers, and identify resources to address these effectively.

Explore innovations in technology, including telehealth and social media, to improve care coordination, patient engagement, and support for lifelong congenital care.

Apply best practices that integrate medical, psychosocial, and societal perspectives, with a focus on specialty-specific care, to ensure successful outcomes for transitioning patients.
