The current problems in practice that we would like to address pertain to a knowledge gap in the most up to date, clinically efficacious, and well supported guidelines for immunizations. Through proper education on this topic, we can better understand the benefits and risks associated with vaccinations which will ultimately contribute to better patient care.

1.) To ensure the safe use of long-acting injectable antipsychotics by distinguishing between currently available agents and formulations and highlighting common medication errors associated with their use.
2.) Review results of July 2023 ISMP/ECRI survey on Drug, Supply, and Equipment Shortages​
3.) Discuss actions to prevent shortages​ while assessing the impact of shortages on patient outcomes, errors, and organizational resources
4.) Discuss UPMC actions to promote medication safety regarding methotrexate use
5.) Compare patient safety event reporting trends in 2022 to prior years
